Home and Community Based Palliative Care Nurse Practitioner The Palliative Medicine Nurse Practitioner provides consultation in palliative medicine to meet the needs of a patient as requested by their referring physician. The nurse practitioner is a specialist in assessing the patient's condition and developing a plan of care to improve patient understanding of their complex illness, adherence to course of treatment, and quality of life in context of patient’s values. The nurse practitioner also develops a plan of care to manage complex disease-related symptoms including emotion, physical, and existential components. Importantly, the nurse practitioner communicates with all members of the patient’s interdisciplinary care team to insure proper coordination of the best care possible.
