A healthcare organization in Marietta, Georgia s actively seeking a compassionate and dedicated Nurse Practitioner (NP) to join their staff as their new Health Guide. In this role, the Health Guide (NP) will assess both behavioral and physical health conditions through a thorough history and physical examination

Responsibilities

• Capturing conditions not yet diagnosed, including by ordering additional studies as indicated
• Organizing the information in a manner that supports successful disability applications
• Conducting visits in the field or in an individual's home when needed
• Making individualized recommendations and/or referrals for programs/care
• Training and advising Guides and Community Resources Guides on clinical issues and treatments
• Supporting Guide Team on care coordination
• Leading care management
• Performing medication reconciliation post-discharge and as needed
• Communicating and coordinating with individuals' care providers, primarily at CMHCs, FQHCs, and hospitals

Qualifications

• Active GA Nurse Practitioner
• 2+ years of Clinical Care experience as a Nurse Practitioner
• Experience working with and managing a population with challenges such as behavioral health and/or SUD conditions
• Experience developing and delivering educational programs to nurses and affiliated medical staff
• Care management and coordination experience
• Intermediate computer skills; familiarity with Microsoft Office Suite
